Our Recuperative Care Services at Fortunate Homes LLC provide temporary, supportive care for individuals healing from an illness, injury, or hospital stay. We provide a secure and structured environment where clients receive important medical and social assistance to help them restore stability before returning to independent living.
We offer the following Recuperative Care Services:
- Basic nursing care, including monitoring overall health conditions and pain management
- Medication assistance
- Patient education
- Immunization assessment and updates
- Clinical goal planning for the recuperative care period and discharge preparation
- Care coordination, including initial evaluation of medical, behavioral, and social needs
- Care plan creation
- Support and referral assistance for legal matters, housing, transportation, case management, and community social resources
- Support and referral assistance for healthcare coverage and other qualified benefits
- Care plan execution, follow-up, and monitoring
- Medical, social, and behavioral (counseling and peer support) services provided within the recuperative care environment
- Community health worker services (Minnesota Statutes PS68.062, 43.25 subdivision 4S, 43.26)
